is there a guide that matches the PBS west/east HD channels on AMC21 125 west?

Yes it looks like you can just go the PBS website and choose the location on the station.
For example http://www.pbs.org/tv_schedules/ then use change provider to view the other PBS schedules.

I found this about a week ago. It seems pretty accurate on the East feed on 103W C band. Haven't checked it against the west feed yet.

PBSHD for PBS East
PBSHDW for PBS West

Other PBS listings I use:
